Here’s how you can participate

Step 1
Tell us about yourself

Click “Count Me In” to complete a simple online form to tell us about yourself.

 

Step 2
Give us permission to collect your samples and data

After you complete the online survey, we'll ask you to fill out an online consent form that requests your permission to obtain copies of your medical records. We'll do the rest, contacting your doctors and hospitals to obtain copies of your records and a portion of your biological samples.

 

Step 3
Learn with us along the way

You'll receive information on how our research can benefit your journey. Throughout the project, we will provide you with regular updates and share the discoveries you have enabled us to make. We may also ask you additional questions about your experience – we want to hear what’s important to you.
